Insurance and Fees

IN NETWORK INSURANCE INFORMATION:

I accept Blue Cross Blue Shield PPO, Care First Blue Cross, HMO, and Federal Plans.  I also accept United Health Care.

Prior to your first session, please download and fill out my INSURANCE VERIFICATION FORM by contacting your insurance company to answer the questions.

OUT OF NETWORK INSURANCE INFORMATION:

Most insurances will reimburse for out of network services, check with your insurance company for those details.

SESSION FEES:

• Initial Intake/Evaluation:  (90 minutes) $175.00
• 55 minute session – $135.00
